Printable Option:   Print the form below, fill it out completely, and then turn the form in at our Records office located at 100 N. Midwest Blvd.

For children under 16 years old, we encourage the printable option. We want to give the child a coupon for a free ice cream cone or donut when the completed form is turned in at our Records office. Limit of one coupon per child; Sorry, adults will not receive a coupon.

Q. Why should I register my bicycle?

A. If your bicycle becomes lost or stolen, you can provide the serial number to the police department when making a report. The serial number helps law enforcement identify lost or stolen items so that they can be returned to their rightful owner.

Q. Where do I find the serial number?

A. Most of the time it is engraved on the underside of the bicycle frame, near the pedals.

Q. What if in the future I no longer own the bicycle?

A. We ask that you please call the phone number below to let us know. This helps us keep our records up-to-date.

If you have any additional questions about this program, please call or leave a message at (405)739-1025.
